##Requirements before class
- Complete all PADI e-Learning | You can purchase the course found [here](https://store.padi.com/en-us/courses/open-water-diver/p/60462-1B2C/?irra=27868)
- Complete eRDp/RDP section that says *Optional*
- Completed Student Medical Statement (if not uploaded digitally)
- Purchase Mask and Snorkel, fins are *optional* ( students must have items for pool session)
- Provide DiVE KS with your Height, Weight, Shoe Size

**Please note we require verification of student information including: height, weight, and shoe size, upon enrollment. This helps us best prepare your equipment for your scheduled program.**

#Student Medical Statement Requirement

This form **must** be completed and submitted to your **PADI dashboard** 3 days prior to class. *We will not accept paper copies.*

If your responses require medical clearance, a signature from a licensed medical professional is required. You will not be permitted to attend your scheduled session without a fully completed form and, if applicable, the required medical clearance on the original last page of the Student Medical Statement. **Do not wait last minute to complete this form. If it is not completed you will not be able to join your scheduled course.**

**Please ensure all documentation is submitted 3 days prior to class. No exceptions will be made.**

##What to bring:
- Mask, Snorkel, Fins
- Swimsuit
- Rash Guard (SPF Shirt) / spandex pants for additional warmth (optional but recommended)
- Towel
- Plastic Water Bottle
- Any and all paper copy forms if applicable

##Aquatics Facility
The pool is located at The Workout Club, 16 Pelham Road, Salem NH. Locker Rooms with showers are available. There are also family rooms available.

##Arriving to class
Upon your arrival you can make your way into the Workout Club building. You can let the front desk know you are there for Scuba Diving Training with DiVE KS. You will need to sign one more waiver upon your arrival. Your instructor will meet you in the lobby from there.
